Modélisation de l’ETP en soins primaires

Development of a model for Therapeutic Patient Education in a multi-professional health centre for people living with a chronic disease.

Project overview The increase in the prevalence of chronic diseases requires effective access to Therapeutic Patient Education (TPE) (1). Currently, TPE is not very accessible for patients and generates social and territorial inequalities in access to care (2). The development of TVE in primary care in the form of a group program appears to be an effective and relevant response because of its accessibility for the population and its proximity to the territories (3). Models for integrating TVE into primary care have been developed internationally (4), but only 20% of TVE programs in Europe were conducted in primary care in 2008 (5). Created in 2008, the Maisons de Santé Pluriprofessionnelles (MSPs), ambulatory practice structures, will be 1889 by 2021. Within them, preventive practice and interprofessional collaboration are particularly developed. Since 2014, some MSPs have been offering FTE programs authorized on the basis of standardized files on a four-year basis and funded by the Regional Health Agencies (ARS). The MSPs' educational team is composed of health professionals (PDS) and sometimes expert patients all trained in ETP. Territorial partners participate in the programs, such as medico-social partners. In France, we have no knowledge of the extent to which MSPs practice TVE programs, nor do we have any descriptive information on their educational content and organization. Our hypothesis is that the practice of TVE programs varies according to the MSP. Favourable and unfavourable characteristics (both educational and organizational) for the design, implementation and maintenance of TVE programs in MSPs, specific to the French health system, probably exist and influence their development. It is important to identify and analyze them in order to improve the support of educational teams in the development of TVE in MSPs. The ultimate goal is to improve the quality and safety of patient care.
