Fertilité et style de vie périconceptionnel

Physical activity, sedentary lifestyle, and body composition as individual periconceptional factors to be considered in the management of idiopathic infertility in couples: Ancillary studies to the ALIFERT and PEPCI research.

Project overview This project is based on the epidemiological case-control study ALIFERT, and the multicenter randomized controlled trial PEPCI, as well as an unsystematic literature review. ALIFERT and PEPCI aim to highlight that certain modifiable factors should be taken into account in the medically assisted reproduction (MAP) of couples with idiopathic infertility. We were particularly interested in the lifestyle of couples and their body composition. Through the ALIFERT study, we have shown that the practice of a reasoned physical activity (neither too high nor too low), the reduction of sedentary lifestyle, and the regulation of body composition are to be considered given their positive association with the fertility of the 143 fertile persons (72 men and 71 women) compared to the 159 infertile persons (79 men and 80 women) studied. These factors would not show the same effects in men and women.
The feasibility of implementing a change in active and sedentary behaviours, associated with addictology, dietetics, and psychology, was tested in a randomized controlled trial (PEPCI) with infertile couples undergoing MAP. An adapted physical activity was proposed to physically inactive and voluntary persons by means of videoconference sessions (Mooven Platform). The study has been completed but the analyses have not yet been conducted. However, we were able to observe that the feasibility of implementing a multidisciplinary intervention in a clinic, co-constructed between health professionals and couples (young workers), remains uncertain. Raising the awareness of professionals in the MPA pathway, treating physicians, and physical activity professionals could be a strategy to enhance the value of this possible non-medicinal intervention, the effectiveness of which remains to be demonstrated.
This research contributes to highlighting that the modifiable factors that are reasoned physical activity, the reduction of sedentary lifestyle, and the regulation of body composition should be taken into account in the MPA of couples with idiopathic infertility. For the first time in France, a paradigm shift in periconceptional support is emerging. It seems important to raise the awareness of the professionals involved in the care and health care process. However, the feasibility of implementing a multimodal intervention in a clinic aimed at modifying the lifestyle of couples should be evaluated. The link between new knowledge and clinical practices should also be questioned, and it should be explored what education is provided to couples on the effect and value of lifestyle change in reducing idiopathic infertility.

I carry out my research in the field of Public Health in order to evaluate - beyond the effect of physical activity - its feasibility, obstacles and facilitators in the field of health.

Activités récentes : Active lifestyle is a complex health behavior, determined by many factors at micro, meso and macro environmental levels. Our current research aims to increase our knowledge of these factors and to explain them within ecological models. Thanks to the experience gained at the Health & Wellness Lab of the University of Calgary (Alberta, Canada) during a CRCT from 2022 to 2023, and based on the results of previous studies, we will be able to implement projects in France based on implementation science, with large samples of patients. We will then be able to document variables that are unfortunately still under-evaluated in multimodal programs promoting physical activity in secondary and tertiary prevention, such as acceptability, adoption, feasibility, fidelity, safety, reach, sustainability, integration into the care pathway, and cost.
